Wait to cast Void Swarm until there are nearby enemies for the Voidling to attack or kill.

Use Call of the Void and Nether Grasp to renew the duration of Malefic Visions on enemies.

Avoiding damage in lane maximizes the uptime on Void Shift, increasing Malzahar's safety dramatically.

Against Malzahar

When Malzahar hits a spell on enemies affected by Malefic Visions, the Visions are refreshed.

Avoid minion waves that have been affected by Malefic Visions. You never know when they may die and pass the Visions onto you.

Malzahar is especially dangerous when he has built up his Void Swarm.

Ability details

Innate: Periodically, Malzahar gains Void Shift until he takes non-minion damage or negates a crowd control effect, to which it then lingers for 0.25 seconds before expiring.

Void Shift: Malzahar gains crowd control immunity and 90% damage reduction.
Void Shift's cooldown refreshes whenever Malzahar takes non-minion damage or is affected by a crowd control effect, and resets upon respawning.

Notes

Void Shift will not resist allied crowd control nor will it be deactivated by them.
Void Shift will not mitigate true damage but the buff will still be consumed.
It does not deactivate against the Nexus Obelisk.
Shields will take priority over Void Shift, but the buff is always consumed, even if the shield fully mitigates the damage.
However, if Malzahar has a spell shield and is hit by an effect that isn't negated by the spell shield, then if the shield fully mitigates the damage, Void Shift will not be consumed.
Spell shields take priority over Void Shift for abilities that contain a crowd control effect.
Void Shift will specifically not deactivate if Malzahar eats Honeyfruit or is within a hostile Frozen Heart passive aura.
Void Shift is special cased to prevent Fizz, and Thresh from attaching to Malzahar.
